The SEND IASS Team would like to collect the views of people who have used our service to help us make sure that we are delivering a service that meets your needs. The questions are only about SEND IASS and not about other services that you might have used.
We use the information you give us to complete reports to the Local Authority, the CCG and the Council for Disabled Children. We also include your views and feedback in our annual report which is published on our Local Offer page.
